Emergency Contact Form

This form is to be used for emergy contact information for all sports camps.  Form must be completed and returned to the South Windsor Parks Recreation Department at least 2 weeks prior to your child beginning camp.  Your child will not be allowed to begin camp until this form is turned in.  Form may be mailed to:
